Final Forms

Every athlete must have a physical completed and uploaded to final forms before they are allowed to participate in any sport.

A physical must be completed and dated after April 15th of every year for the physical to be good for the entire following school year.

For example, a physical dated April 15, 2024 or later will be good for the entire 2024-2025 school year.
